Registration fees include: Delegate - Welcome reception - Attendance in all conference sessions - Conference kit - Coffee breaks Accompanying person: - Welcome reception - Closing ceremony Registration Instructions a. Download the Registration Form by clicking here b. Accomplish the Registration Form c. Send the file as an attachment to the 7th APSSAM Conference Secretariat at apssam08@yahoo.com. Don't forget to type 'REGISTRATION' as the subject Field trip (fees include packed lunch and transportation) Rural Health Center $ 15.00 Hospital facility $15.00 Deadlines: EARLY BIRD: until June 15, 2008 ON-LINE REGISTRATION: June 16, 2008 to date of conference Registration Acknowledgment Your registration will be acknowledged electronically or via facsimile within a week of receipt of registration form. Registration fees will be calculated based on the date of the receipt of payment. Cancellation & Refund Cancellation should be made in writing (stating your name and reference number) and sent to 7th APSSAM Conference Secretariat at apssam08@yahoo.com In case of Cancellation: a. Before 31 May 2008 you will receive a full refund less a deduction of USD 50.00 for administrative cost. b. Between 1st June, 2008 and 01 September, 2008, you will receive a refund of 50 % of your registration fees. c. No refund will be made for cancellation after 01 September, 2008. |
